The following office locations offer Driver Records, Crash Reports, and Driver License Reinstatements:

Reinstatement Fees

  • Suspended/cancelled licenses - $100
  • Revoked licenses - $175

Alcohol/drug-related offenses:

  • Suspended - $275
  • Revoked - $275
  • Additional drug-related fee - $25

Other Fees:

  • Fail to surrender license within 30 days - $50
  • Fail to pay child support - $50

Driver License Reinstatements must be performed at one of the Driver License Offices listed above.

Driver Record Fees

  • Copy of Driver Record (MVR) - $5.75

Driving Records may be purchased through the mail using the Driver Record Request Form located on our Forms page, or in person at any of the Driver License Offices listed above.

Crash Report Fees

  • Copy of Crash Report - $15

Crash Reports may be purchased through the mail using the Crash Report Request Form located on our Forms page, or in person at any of the Driver License Offices listed above.

NOTE: NO PERSONAL CHECKS ARE ACCEPTED.
